Important information about your Dentiny membership
Not Insurance
Dentiny is a dental membership program, not insurance. We are not an insurance company and do not provide insurance coverage. This is a direct care membership that provides access to preventive and common dental procedures at a fixed monthly rate. We do not file insurance claims, and services are not covered by insurance.
Scope of Services
Dentiny provides access to general dentistry services including: regular exams, cleanings, diagnostic X-rays, fillings, crowns, extractions, and root canals. All covered services are provided at no additional cost to members beyond their monthly membership fee of $72 per member per month.
Exclusions
The following services are NOT included and are not covered by Dentiny membership:
- Dental implants
- Orthodontics (braces, Invisalign, etc.)
- Periodontal surgery
- Cosmetic procedures (teeth whitening, veneers, etc.)
For excluded services, we can provide referrals to qualified specialists. Patients are responsible for all costs associated with excluded services and may use their existing dental insurance or pay out-of-pocket.
Scheduling
Appointments are subject to availability. We prioritize employer groups and work to accommodate scheduling preferences. During peak times, some flexibility may be required. Members can book appointments through our online scheduling system or by calling directly.
Membership Period
Membership is billed monthly at $72 per member and continues until cancelled. If the employer pays, cancellation requires 30 days' written notice from the employer. If employees pay via payroll deduction, employees may cancel with appropriate notice to their payroll provider. Membership fees are non-refundable after services have been rendered.
Payment
Membership is $72 per member per month. Employers can choose to pay in full for all employees, or set up payroll deduction so employees pay directly. All payments are due monthly as agreed in your enrollment contract. Late payments may result in suspension of services.
Refunds
Membership fees are non-refundable after services have been provided. If services have not been rendered, refunds may be available on a case-by-case basis. Please contact us to discuss refund requests.
Clinical Discretion
All treatment decisions are made by licensed dental professionals based on clinical judgment and patient needs. Dentiny providers reserve the right to determine appropriate treatment plans, including when referrals to specialists are necessary. Treatment recommendations are based on professional standards and individual patient circumstances.
Referrals
For services not included in membership (implants, orthodontics, etc.), we will provide referrals to qualified specialists. Members are responsible for all costs associated with referred services. Members may use their existing dental insurance or pay out-of-pocket for referred services.
Privacy
We take patient privacy seriously and comply with HIPAA regulations regarding protected health information. Patient records are maintained securely, and information is only shared as necessary for treatment or as required by law. For more information about our privacy practices, please contact us.
Changes to Terms
Dentiny reserves the right to modify these terms with 30 days' notice to members. Members will be notified of any material changes via email or through their employer. Continued use of services after changes take effect constitutes acceptance of the modified terms.
